We treat patients with the following types of coverage: Our office will file your insurance at no charge. It is to be understood and agreed that services rendered are charged to you directly and you are personally responsible. We are not certain if your insurance covers chiropractic, although most policies do provide coverage. The amount they pay varies from one policy to another, ranging from 100% coverage to a small percentage. Because of this difference between policies, we request that each patient who wishes to file insurance claims through this office, pay the insurance policy deductible and the patient's percentage or co-pay. Your insurance will be filed by the fastest most efficient method available, either by electronic submission or computerized forms. When all insurance checks have been received, we will refund any overpayment to you. We welcome Medicare patients, but do not accept assignment from Medicare. We are Participating Providers, which means services are discounted for Medicare patients and Medicare checks come directly to our office. The Medicare patient is responsible for payment in full for the first visit and for any other charges which are not covered by Medicare. We will complete any necessary forms and file them with Medicare and any secondary insurance at no charge. This clinic welcomes and accepts Medicaid patients. You will be asked to present your current Medicaid Assistance Card to the receptionist each month. Our office will submit the forms necessary for reimbursement of your chiropractic care. A co-payment may be required at the time of service. Most managed care plans require the patient to pay a co-payment or percentage at each office visit. Since we are added to others frequently, please call to verify coverage for your plan. Even if we are not a provider for your plan, most plans offer good out-of-network coverage. Medical treatment for injuries occurring at work is covered 100% by Workers' Compensation. Such coverage includes treatment by medical or chirorpactic physicians in Nebraska. Our office will handle all necessary forms with your employer, worker's compensation carrier, and the State of Nebraska. Personal Injury or Automobile Accidents Chiropractic treatment of injuries from an automobile or other accident is covered by liability insurance. Coverage for automobile injuries is paid by the auto insurance of the other party or by the medical coverage portion of your own auto policy. Additional coverage may be available through your health insurance policy. Flexible Spending Accounts or HSAs (Health Savings Account) All out-of-pocket expenses for chiropractic are qualified expenses through Flex Plans, Cafeteria Plans or HSAs. Tax-free funds from these accounts may be used for co-pays, co-insurance (patient's percentage), deductibles, or any other chiropractic expenses not covered by insurance. Time-of-Service (TOS) Discount: For those patients who choose to
pay 100% of the fee at the time the service is provided, a 15% discount is
applied to payments received. Any financial class of patient may opt to take
advantage of the TOS Discount, whether your case is covered by health
insurance, Workers’ Comp, Personal Injury, or Medicare. |
